Hayley runs a Dietetic Clinic at Nethercotts Opticians in Tadworth on Saturday mornings from 9.00 - 13.00.
Initial assessments are for one hour, and follow up appointments are normally thirty minutes unless otherwise requested.
Appointments will include consideration of current situation, medical history, medication, relevant test results, dietary preferences and culture, to help create a personalised nutrition plan. Information sheets will be provided.
Referrals are accepted from GPs, Consultants and Allied Health Care Professionals. Self referrals can also be accepted.
For referrers- please include medical history, current medication, results of any relevant investigations including blood tests.
Home visits can be arranged if required for an extra charge depending on location.
